I do have batch information for the first 4 Pfizer trial batches of which at least 2 can be linked to UK but it might be better to email me if you want the data and in fact this info is probably earlier than Dec 2020, but please have it if you feel it will help.
I recently discovered that an acquaintance has died. I suspected he had. Our last conversation was of him telling me that he'd had the jab and hadn't felt well since. His last words to me were 'I'm fx'd, Rebecca, I'm fx'd.' I know another man who recently had a heart attack - c. 60 but a wiry, fit guy. Jabbed. I don't know them well enough to fill in the survey.
Maybe not your bailiwick, but I'm wondering if there is manipulation in the US excess all cause mortality data by the CDC re-apportioning "expected" deaths amongst the different age cohorts to hide the excess youth deaths in with the higher number of elderly deaths.

The person I know was a close friends family member in Slovakia, so not relevant for this. However I will add he was healthy late twenties and died of heart inflammation complications post second mRNA vax.
It would be interesting to do this for different countries and see if there is any pattern between them. Unfortunately, here in Canada, I know of two people locally, both in their 30s just starting their families, and both died of heart failure within days of their second jab, leaving behind their spouses and children. It is completely obvious to me and a few others what happened to them but I cannot believe how many of our friends and family would never ever attribute it to the jab. It’s astounding.
According to Norman Fenton. Zero children died in 2020 where only Covid appears on the death certificate and 2 in 2021 using data from FoI and ONS----but you probably know that

Whilst every death is tragic, the ONS reports that only two children under 18 died from Covid during 2021
In Scotland, no healthy person under the age of 19 has died of Covid.
In N.Ireland, no healthy child has died of Covid.

Why do you want to know the vax status of the person filling in the questionnaire, please? Somebody told me that she knew of 10 people who had died after the jab but wouldn't complete the questionnaire because of the last question.

To add, my parents went together to get jabbed, so likely same batches. My dad got shingles after Pfizer. South Buckinghamshire
They are pushing the shingles vaccine like CRAZY in my area (in the US). Terrifying graphic TV commercials, texts and e-mails from CVS etc.

Naomi Wolf’s book goes into great detail about how they have misled us with manipulated data. It is shocking dishonesty on multiple fronts.
